D13 (red led) continuously glowing with wall adapter and otg usb both

Dear Gerald,

  I was recently working with my Beagleboard-xm RevC and after
switching from otg power to wall adapter power supply, all of a sudden
red led D13 started glowing continuously.
After then i plugged back the otg power to find the same situation.
I checked the test points given in manual. expect AUX _3.3v and
VBUS_5v all are working fine with otg power. and none are working
with wall power adapter.

What do you think the Overvoltage protection IC (NCP349) is
damaged ?

I have exact same issue. In my case, D13 is always on when using otg
USB OR wall adapter. Just sent RMA request.

You have a grounding issue. This is a commom problem when trying to run from OTG. In some cases the ground is not actually earth ground inside the PC. You need to make sure that the DVI-D l port is connected tand the monitor that is connected has same earth ground as the PC itself. Sometimes if you have the serial port connected that will work as well. This is not something that can be fixed via an RMA.

Thank you for your reply, But sir i have tried it everywhere in my
college lab and few other places. It worked before but not now...

OK. Then the RMA process is the correct route. Getting it out of your lab is the right way to verfy it.

The grounding issue only applies to OTG? If only DC supply is plugged
in and D13 is glowing, do it mean some power circuit is bad? My power
supply works well with beagleboard before, and it's 5.19 v.

Well my 90 days period has expired and I was not able to send it
because of my semester end exams. Is there any chance they might
accept my RMA request?
